Continuum of Care for Stroke Patients

Transitioning from rehab to the community is a challenge for stroke patients. People who have had a stroke, their relatives, and their caregivers need to adapt to a new life routine. Their acquired condition requires a new set of abilities to execute daily self-care activities. The rehabilitation process in the hospital should be able to help patients to be independent after discharge, but that is not always the case.

In partnership with Bridgepoint Active Healthcare, we did thorough academic and field research along with a co-design workshop involving several stakeholders impacted by this complex problem. Based on this study we proposed a system to help address the individual needs of the stroke patients going through this transition.

Service blueprint

This is the end result of the project: a service blueprint describing how the different touchpoints of the Continuum Care service help stroke victims in various areas during their transition to home.
